Professional Development Incentive Award-Higher Education Application
Higher Education Please fill out this form prior to your request for payment if your college credit courses have changed since the time you were approved for Higher Education Merit Pay.
If
you have been approved for Spring semester Higher Education Merit Pay
and have completed your approved coursework please submit the following
items: 1.
A copy of you Spring semester transcripts with verification of
successful completion of approved courses

Merit Pay I Award Participants If you have been approved for a Merit Pay I award, upon completion of your required approved training hours, which must be completed between 8/1/2009-8/31/2010, please submit the following items to the Early Childhood Project to receive your award money. 1. A copy of your online Professional Development Record and any training certificates* for training that does not appear on the record 2. A completed W-9 tax form-please fill out your Social Security Number and not the Employer Identification Number when you complete! (download below) 3. A copy of your current Practitioner Registry Certificate

*Need help finding your Professional Development Record? Since you must be active on The Practitioner Registry to claim your award, just go to our homepage, click on Practitioner Registry, then look for the blue link at the top of the page to view your Practitioner Registry record. Once you have signed in with your Registry ID and password, go to the last box on the left side of the screen and click on "ECP Reports" You can then click on "Professional Development Record." Print this off and send it to us.
